I confirm, as the authorised representative of the Supplier, that the lamp (or lamps) listed above:

is/are compatible with the extra low voltage transformer/s selected, as demonstrated during limited testing; and
during testing with the listed transformers, the lamp had stable light output, no sign of flicker and no audible noise;
that the performance of the transformers and lamps during testing does not guarantee the performance of the lamp (or lamps) listed above will operate in conditions different from testing conditions.

I consent, as authorised representative of the Supplier to:

the Department publishing the name of the supplier, LED lamps details and Transformers details entered into this form on the Energy Rating website, www.energyrating.gov.au
the Department making available the name of the supplier, LED lamps details and transformers details entered into this form on the Department’s Light Bulb Saver app
the Department using relevant information from this form to populate a GEMS registration application form for the identified lamp/s, in the event that LED lamps are regulated, and this functionality is developed with the registration system, in the future.
